AFTER THE ENROLLMENT APPLICATION HAS BEEN COMPLETED, please submit your curriculum plan.

You may submit your plan after your welcome call or if you have your plan in place, you are welcome to complete it once your enrollment application has been completed and fees paid. 

We will discuss curriculum during the welcome call. This form is required for enrollment. Thank you!
